Job Description:
HCA Healthcare is seeking a dedicated and compassionate Home
Health PRN Physical Therapist to join our dynamic team. In this
role, you will have the opportunity to work closely with patients
in their homes, providing personalized physical therapy services
that adhere to our high standards of care. This position is ideal

Evaluate patient conditions and develop appropriate physical
therapy treatment plans. Implement patient care programs that focus
on achieving goals set in tandem with patients and their families.
Document patient progress and communicate with healthcare team
members to deliver consistent care. Educate patients and families
about health condition and maintenance techniques. Assess the
outcomes of physical therapy interventions and adjust plans as
needed. Strong interpersonal and communication skills. Ability to
work independently and manage time effectively. Proficiency in
creating and following individualized care plans. Capability to
perform physical tasks and lift weights as necessary for patient
care. Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Current state licensure as a Physical Therapist. CPR
certified. Valid driver’s license and reliable transportation.
