Encompass Health Corporation, a leader in integrated healthcare services, is seeking a dedicated Case Manager II to join our dynamic team. This role involves comprehensive management of patient care across our network of inpatient rehabilitation hospitals and home health agencies. The successful candidate will coordinate, negotiate, and manage the care of patients to facilitate the achievement of quality, cost-effective outcomes. Working closely with healthcare professionals and administrative staff, the Case Manager II plays a critical role in enhancing patient outcomes while maintaining efficient use of healthcare resources. Develop and implement individualized care plans for patients, coordinating with healthcare providers and ensuring continuity of care. Monitor patient progress and adjust care plans as necessary. Facilitate multi-disciplinary team meetings to review treatment goals, utilizing cost-effective resources. Advocate for patients' needs and preferences with healthcare providers and insurers. Educate patients and families on the healthcare process, illnesses/injuries, and medication management. Maintain comprehensive documentation of patient interactions, services provided, and outcomes. Strong organizational and communication skills Ability to multitask and manage multiple cases simultaneously Experience with healthcare databases and patient management software Knowledge of healthcare regulations, including HIPAA Skills in negotiation and conflict resolution Medical Insurance Dental Insurance Vision Insurance 401k Certification in Case Management (CCM) preferred.
