POSITION SUMMARY Provide pharmaceutical care to the patients in the
hospital through supervision of and participation in the drug
distribution system screening patients' drug therapy for overdoses
or under doses duplication of therapy, drug interactions, drug
allergies appropriateness of therapy and conferring with other
healthcare professionals and patients about drug therapy. These
functions are performed in order to ensure that the highest
possible quality of medication therapy is provided to the patients
of this hospital. POSITION REQUIREMENTS Formal Education /
Training: Earned a Pharmacy diploma or degree from a college or
university. Must have a current license from the State of Tennessee
or eligible. Must comply with all continuing education requirements
of the state. ACLS within 6 months of hire. Workplace Experience:
Experience in Unit dose and IV Admixtures desirable. Equipment and
Skills Training: Must work with personal computer, laminar airflow
hood, chemotherapy hood, pharmaceutical balance, medication
prepackaging system, calculator, telephone, copier, FAX machine,
computer order entry pharmacy computer and pharmacy automation.
Physical Environment: Ability to handle a high volume of work with
acceptable speed and accuracy. Ability to handle frequent
interruptions. Able to respond quickly to emergency requests.
Ability to set priorities work independently and work cooperatively
with hospital and pharmacy staff. Physical Effort: Will be required
to sit at computer desk and stand at various work areas for lengthy
periods of time. Efficiently communicate both orally and in
writing. Safely and efficiently stand, stoop, bend, reach and lift
in order to maintain the department equipment and supplies. Push
wheeled carts weighing approximately twenty pounds to nursing
units. Must have the ability to remain mentally alert and maintain
concentration with attention to detail for prolonged periods.
Ability to handle frequent interruptions and adapt to changes in
workload and work schedule. Ability to set priorities and solve
problems. Occupational Hazards: Potential for exposure to hazardous
and toxic substances (including chemotherapeutic, cytotoxic drugs
and cleaning solutions), sticks or cuts by needles and other sharp
items. Potential for musculoskeletal injuries if proper lifting and
carrying techniques are not used. Customer Service: Must practice
Williamson Health's Standards of Performance Service Guidelines and
Standards. Must be sensitive to the needs of patients, families,
visitors, co-workers and staff. Must possess an extremely pleasant,
courteous and helpful attitude; excellent communication skills and
the ability to work closely and cooperatively with others is
required. Fosters a team environment by providing orientation and
training to new team members. Ability to communicate effectively
(verbal and written). Ability to meet the pharmaceutical care needs
of neonatal, pediatric, adolescent, adult, geriatric, obstetric,
and oncology. PERFORMANCE STANDARDS Monitors drug therapy to
evaluate appropriateness of use, dose, dosage form, regimen, route,
therapeutic duplication, and drug interactions, allergy screening
and other potential problems. Accurately provides computer order
entry for medications and IV admixtures Ensures drugs are prepared,
labeled and dispensed accurately and in a timely manner.
Understands USP797 and using proper aseptic technique prepare and
checks all doses of intravenous (IV) admixtures prepared by
technicians Approaches work assignments with a positive attitude.
Conforms to personal device usage policy. Provides Pharmacokinetic
and clinical consultations (renal dosing and IV to PO etc) and
clarification to practitioners and documents recommendations and
follow up Detects, monitors, documents and reports adverse drug
reactions and medication errors using order entry system and
clinical surveillance reporting system, routes to appropriate
personnel. Provides accurate, adequate, and timely drug information
to health care professionals and patients Monitors for formulary
compliance, prescribing criteria, and suggests, appropriate cost
effective therapeutic alternatives to the medical staff when
appropriate. Pharmacist provides presentations, publications and
other informative activities on drug related topics to pharmacy,
hospital, and community. Differentiate age specific medications and
dosages for neonates pediatrics (0-14yr), adult (15-64yr) and
geriatric (65yr ). Understands Pharmacy-related Compliance
