Job Description:
TriStar Centennial Medical Center, a renowned healthcare
facility in Nashville, Tennessee, seeks a dedicated BMT Case
Manager RN to join our dynamic team. As a leader in complex
healthcare, we specialize in oncology and a variety of other
high-end medical services. The BMT Case Manager RN will be crucial
in managing patient care in the Bone Marrow Transplantation (BMT)
unit. This role requires an efficient, compassionate, and
knowledgeable professional who will coordinate and streamline care
delivery, ensuring optimal outcomes for transplant patients. The
responsibility includes working closely with a multidisciplinary
team to tailor recovery plans, facilitating communication between
patients and various healthcare providers, and providing emotional
and educational support to patients and their families through the
BMT process. Assess patient needs and develop individualized care
plans in collaboration with other healthcare professionals.
Coordinate with physicians, nurses, and other healthcare providers
to ensure the effectiveness of the treatment plan. Educate patients
and families about the transplantation process and aftercare
requirements. Monitor patient progress and adjust care plans as
needed. Ensure that healthcare services are delivered in compliance
with state and federal regulations. Maintain comprehensive
documentation of patient care and communication. Serve as the
primary point of contact for all parties involved in the patient's
care. Strong knowledge of bone marrow transplantation and oncology
nursing. Excellent communication and interpersonal skills. Ability
to handle high-stress situations. Proficiency in clinical
management software and patient care systems. Compassionate and
patient-focused approach. Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Registered Nurse (RN) licensed to
practice in the state of Tennessee. Certification in Case
Management (CCM) preferred. BLS and possibly ACLS
certification.
