Job Description:
Aveanna Healthcare, a leader in home healthcare services, is
seeking a dedicated Home Health LPN Clinical Coordinator to join
our team. This role is pivotal in providing exceptional nursing
care and coordination services for our patients in a home setting.
The coordinator will oversee patient care plans, collaborate with
healthcare providers, and ensure compliance with all local and
federal regulations. A focus on patient-centered care is essential,
as is ensuring the highest standards of safety and effectiveness of
home healthcare services. Oversee the preparation and management of
patient care plans. Collaborate with doctors, nurses, and other
healthcare professionals to ensure comprehensive patient care.
Monitor patient progress and adjust care plans as necessary. Ensure
compliance with all healthcare laws, policies, and regulations.
Train and mentor new staff and provide ongoing education to all
team members. Handle patient queries and concerns efficiently and
compassionately. Strong knowledge of nursing practices and
principles. Excellent organizational and time management skills.
Effective communication and interpersonal skills. Proficiency in
electronic health record systems. Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Valid LPN license.
CPR certification.
