Schedule About the Role We are seeking a skilled and compassionate
Physical Therapist (PT) to join our home health team. In this role,
you will provide one-on-one therapy services in the comfort of
patients’ homes, helping them recover, regain independence, and
improve their quality of life. You’ll work with a diverse patient
population, tailoring treatment plans to meet individual goals and
functional needs. Key Responsibilities Conduct evaluations to
assess patient needs and create individualized treatment plans.
Provide therapeutic interventions focused on mobility, strength,
balance, and functional independence. Educate patients and families
on safe practices, exercises, and home modifications. Collaborate
with an interdisciplinary care team to ensure comprehensive patient
care. Accurately document patient progress and update care plans as
needed. What We’re Looking For Current state licensure as a
Physical Therapist (PT). Strong clinical and assessment skills.
Ability to work independently and manage a flexible schedule.
Compassionate, patient-centered approach to care. Prior home health
