Job Description:

Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance. -Provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.Documents patient care given.Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.Works as an advocate for the physical and emotional well-being of the patient.Establishes and maintains efficient methods of ensuring the medical necessity and appropriateness of all hospital admissions.Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of medical care, including admissions, length of stay, transfer, and discharge.Monitor's patient and family satisfaction.Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources.Provides discharge education and resource referrals to patients.Performs chart reviews to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement. -Perform other duties as assigned. -May be required to perform duties of the CNA, MA or LPN or other nursing staff on occasion. -Attends and participates in staff meetings, in-service, projects and committees as assigned. -Adheres to and supports policies and procedures of the STRHS. -Works scheduled shifts including overtime, when necessary. -Accepts all call schedule as directed. -Maintains a neat and professional appearance in compliance to the existing dress code. -While performing this job, occupational exposure to bloodborne pathogens is present for all employees -Compliance to the STRHS Standard of Behavior is required. -Compliance to Dress Code Standards. -Should this job description be filled by a PRN employee, they will be subject to the requirements of that position. -Reports to the Department Director.
Job RequirementsMinimum EducationAssociate's degree required. -Required SkillsRequires critical thinking skills, decisive judgment and the ability to work with minimal supervision.Must be able to work in a stressful environment and take appropriate action. -CertificationsAHA Basic Life Support (BLS), within 30 days of hire. - -Licenses -Current and active Registered Nurse Licensure for the State of Tennessee.
